  1 If the amount of contribution varied by either size or composition of
family, the rate for an employee with a spouse and one child was used. 
For a small percentage of employees, the employee contributes the same
amount for single and family coverage.

Collection was conducted between February and December 2000.  The average
reference period was July 2000.
